مقايسه ي فيلم هاي پرتونگاري معمولي و خود ظهور (Self developing) در برآورد طول كاركرد كانال

بيان مساله: تعيين طول كاركرد كانال اهميت ويژه اي در موفقيت درمان هاي ريشه دارد. روش هاي گوناگوني به اين منظور مورد استفاده قرار مي گيرد و در اين ميان پرتونگاري شايع ترين روش است. هدف: هدف از پژوهش كنوني، مقايسه ي فيلم هاي معمولي و خود ظهور (Self developing) نسبت به روش استاندارد طلايي در برآورد طول كاركرد كانال بود. مواد و روش: در اين بررسي توصيفي به منظور تعيين طول كاركرد كانال، با دو گونه فيلم معمولي و خود ظهور در شرايط آزمايشگاهي از 45 كانال مزيوباكال دندان هاي مولر نخست فك بالا كه در فانتوم هد مانت شدند پرتونگاري فراهم گرديد. فيلم هاي معمولي در پروسسور خودكار ظاهر و فيلم هاي خود ظهور بر پايه ي روش پيشنهادي كارخانه ي سازنده آماده شدند. تصاوير توسط سه اندودونتيست و يك راديولوژيست به گونه ي دو سويه كور (Double blind) ارزيابي گرديد. طول كاركرد حقيقي (نمايه طلايي) بر روي دندان هاي كشيده شده مشخص شد و سپس اختلاف ميان طول كاركرد در دو روش پرتونگاري با يكديگر و با طول كاركرد حقيقي تعيين گرديد .نتايج با آزمون هاي آماري تي (t) و تي زوج (Paired t) ارزيابي شد. يافته ها: ميانگين تفاوت طول كاركرد ميان روش پرتونگاري با فيلم معمولي و نمايه ي طلايي برابر با 33/0 ميلي متر و ميانگين تفاوت طول كاركرد ميان روش پرتونگاري با فيلم هاي خود ظهور و نمايه ي طلايي 14/0 ميلي متر بود، كه اين اختلاف از نظر آماري معنادار نبود (06/0 = p ) و (7/0= p ). همچنين اختلاف آماري معنادار ميان دو روش پرتونگاري در برآورد طول كاركرد كانال ديده نشد (4/0= p ). نتيجه گيري: گرچه اختلاف معنادار ميان اين دو گونه فيلم در تعيين طول كاركرد وجود نداشت، بررسي ها نشان مي دهد، كه فيلم هاي خود ظهور نتايج آماري بهتري دارند. بنابراين، در شرايطي كه به تاريكخانه و محلول ظهور و ثبوت دسترسي نيست، مي توان از اين گونه فيلم به جاي فيلم معمولي استفاده كرد. واژگان كليدي: طول كاركرد، كانال ريشه، فيلم خود ظهور، فيلم پرتونگاري معمولي

Statement of Problem: Determining the appropriate Working Length (WL) is an important factor in the success of root canal therapy. Several methods are available for this purpose but radiography is the mostly used one. Purpose: The aim of this study was to compare the conventional and self -developing films with a Gold standard in estimation of the working length. Materials and Method: In this descriptive study, radiographs were obtained with conventional and self-developing films from 45 mesiobuccal maxillary molar root canals which were mounted on a phantom head. Conventional films were processed automatically and self-developing films manually in accordance with manufacturer’s instructions. The radiographs were examined by three endodontologists and one oral and maxillofacial radiologist under double-blind conditions. Actual working lengths were measured on each tooth (Gold standard) and then measurements from each film type were compared together and against the Gold Standard. The data were analyzed using t-test and paired t-test. Results: The average difference in WL between the conventional and self-developing films measured against the Gold standard was 0.33 mm and 0.14 mm, respectively. No significant difference was found between these two types of films and the Gold standard ( p =0.06 and p =0.7, respectively). Also, there was no significant difference between these two types of films for estimation of the working length ( p =0.4). Conclusion: Although there was no significant difference between these two types of films, the assessment shows that self-developing films produced more accurate results in estimation of the working length. It is concluded that in the absence of dark room and processing solutions, self-developing films can be suitably used instead of conventional films.
