Sarcomes radio-induits après cancer du sein : expérience de lʹinstitut Curie et revue de la littérature

Résumé iothérapie adjuvante joue un rôle essentiel dans la prévention des récidives locorégionales du cancer du sein et son efficacité a été démontrée dans plusieurs essais randomisés. Les sarcomes radio-induits, complication exceptionnelle de la radiothérapie, sont grevés dʹun pronostic sombre. Le premier cas rapporté dans la littérature est un ostéosarcome pariétal consécutif à une radiothérapie mammaire, décrit par Beck en 1922. En 1948, Cahan et al. ont défini des critères diagnostiques du sarcome radio-induit. Depuis, on retrouve des publications rapportant des sarcomes survenus après traitement de cancers mammaires et gynécologiques, de maladies de Hodgkin et dʹautres types de cancer. Cet article rapporte lʹexpérience des sarcomes radio-induits survenus chez des patientes traitées pour un cancer du sein à lʹinstitut Curie. Lʹincidence, lʹhistologie, les délais dʹapparition, le traitement, le pronostic ainsi que quelques aspects radiobiologiques de ces tumeurs rares seront ensuite abordés et comparés aux données de la littérature. nt radiotherapy to the breast plays a significant role in preventing local failure in women treated for early stage breast cancer. This fact is supported by multiple clinical trials demonstrating that adjuvant radiotherapy decreases the risk of local recurrence and increases the rate of breast preservation, and actually the rules of adjuvant breast irradiation are clearly established. Sarcomas are a rare but recognized complication of radiation therapy for breast carcinoma, and are associated with poor prognosis. The first case of a bone sarcoma after radiation therapy of breast cancer was described by Beck in 1922. In 1948, Cahan et al. defined the criteria for diagnosis of radiation-induced sarcoma. Since then, some studies have reported the incidence of radiation-induced sarcoma after radiotherapy for different cancers. This article reports and discusses the incidence, management and treatment outcome of radiation-induced sarcomas occurring after radiotherapy for breast cancer in our institute. The incidence, histology, latency of appearance, genesis, their treatment and the prognostic factors of these rare tumors are discussed and the literature is reviewed.
