Chimiothérapie d’induction des carcinomes épidermoïdes de la tête et du cou

Résumé itement de référence des carcinomes épidermoïdes inopérables de la tête et du cou repose sur l’association d’une radiothérapie avec un sel de platine. Les résultats obtenus sont largement perfectibles, puisqu’un patient sur trois environ est en vie cinq ans après le diagnostic. La chimiothérapie d’induction a bénéficié d’un regain d’intérêt depuis une dizaine d’années, après l’introduction des taxanes en association avec le cisplatine et le 5-fluoro-uracile. La triple association taxane–cisplatine–5-fluoro-uracile utilisée en induction a démontré sa supériorité sur celle cisplatine–5-fluoro-uracile en termes de survie. La généralisation de l’association taxane–cisplatine–5-fluoro-uracile est limitée par sa toxicité et l’absence d’essai comparatif comportant un groupe de référence avec une chimioradiothérapie classique utilisant les sels de platine aux doses optimales. En l’attente de publication de plusieurs essais de phase III, le choix entre une chimiothérapie d’induction suivie d’une chimioradiothérapie et d’une chimioradiothérapie d’emblée se fait au cas par cas en tenant compte de l’état physiologique du patient, de la capacité de l’équipe médicale à gérer un protocole d’induction lourd et des caractéristiques anatomocliniques de la tumeur. andard treatment for head and neck inoperable squamous cell carcinoma is an association of radiotherapy and platinum. Until the results of new phase III trials are published, the choice between induction chemotherapy followed by concomitant chemoradiotherapy or concomitant chemoradiotherapy alone has to be made on an individualized basis, taking into account the patientʹs medical condition, the ability of the medical team to deal with intensive treatment regimens, and the clinical/pathological characteristics of the tumour.
