Radiothérapie conformationnelle avec modulation d’intensité dans les cancers de la sphère ORL : éthique et sélection des patients

RésuméObjectif de l’étude iothérapie conformationnelle avec modulation d’intensité (RCMI) permet une meilleure couverture des volumes cibles et une meilleure épargne des organes à risque avec une diminution de toxicité dans les tumeurs des voies aérodigestives supérieures. Elle nécessite plus de moyens humains et matériels que la radiothérapie conformationnelle. S’ils sont insuffisants, une sélection des patients recevant la RCMI peut être nécessaire, posant un problème éthique. el et méthodes vons recueilli les motivations de choix de technique de radiothérapie pour les tumeurs des voies aérodigestives supérieures durant un mois de pénurie de physiciens. ats uf patients ont reçu une RCMI et huit une radiothérapie conformationnelle. La radiothérapie conformationnelle a été choisie pour les traitements palliatifs, des irradiations postopératoires nécessitant un début rapide, des tumeurs laryngées et une ré-irradiation. La RCMI a été préférée pour les volumes cibles complexes et les localisations provoquant des xérostomies importantes en radiothérapie conformationnelle standard. Ces choix ont été confrontés aux critères de bioéthique de Beauchamp et Childress. La bienfaisance justifiait la RCMI ou la radiothérapie conformationnelle selon les circonstances. La non-malfaisance cherchait à diminuer la toxicité de la RCMI. La justice était remise en question par la sélection.
