Curiethérapie du cancer du sein

Résumé iethérapie interstitielle reste une technique adaptée pour délivrer une forte dose (augmentation du taux de contrôle local) dans un petit volume (faible toxicité) dans le cadre de l’irradiation du cancer du sein. Dans cet article sont présentées les données techniques relatives au déroulement d’une curiethérapie du sein allant de l’implantation du matériel vecteur jusqu’au traitement proprement dit en passant par la planification. Les différentes indications de la curiethérapie du sein sont toutes les indications d’une irradiation mammaire focalisée sur le lit tumoral initial (irradiation partielle du sein) que sont le complément d’irradiation après radiothérapie externe (boost), l’irradiation partielle et accélérée du sein et le second traitement conservateur radiochirurgical en cas de rechute locale homolatérale. Les résultats en termes d’efficacité et de toxicité sont décrits pour chaque indication. In this article, we present the technical data related to the breast brachytherapy proceeding, from the implantation of the vectors to the treatment itself, including the planning. The different indications of breast brachytherapy include all the breast irradiations focusing on the initial tumour bed (partial irradiation of the breast), such as boost, accelerated and partial breast irradiation (APBI) and second conservative radiosurgical treatment in case of ipsilateral in-breast recurrence. The results in terms of efficacy and toxicity are presented for each indication. Interstitial breast brachytherapy, performed according with the standard rules, remains a major technique for breast cancer treatment.
