Abstract :
Objective- The objective of the present study was to determine the changes in heart rate, respiratory rate and arterial Oxygen saturation (SpO2) by administration of three drug combinations during upper gastrointestinal endoscopy. Design- Experimental study. Animals- 21 healthy and adults dog were randomly divided into three groups. Procedures- The IV combination of 6.5 mg/kg ketamine and 0.2 mg/kg diazepam was administered in group KD and 6.5 mg/kg ketamine and 0.3 mg/kg midazolam and 6.5 mg/kg ketamine and 0.4 mg/kg medetomidine were used in group KMi and KMed respectively. Respiratory rate/min, heart rate/min and SpO2 were recorded prior to induction of anesthesia and during endoscopy in esophagus, cardiac level, cardia and during exertion. In addition time to induction of anesthesia and duration of anesthesia were recorded in all dogs. Results- Time to induction of anesthesia and duration of anesthesia were significantly shorter in group KMi compared to group KD and KMed (P<0.05). All of the dogs were suffered from hypoxia. However the changes were significant in groups KD and KMi (P<0.05). Conclusion and Clinical Relevance- All combinations of the drugs produced hypoxia, however the hypoxia was less when the combination of ketamine and medetomidine was used.Oxygen supplementation is recommended during upper gastrointestinal endoscopy in dogs to prevent hypoxia.

ﻫﺪف- ﻫﺪف از اﻳﻦ ﻣﻄﺎﻟﻌﻪ ﺑﺮرﺳﻲ ﺗﻐﻴﻴﺮات ﺿﺮﺑﺎن ﻗﻠﺐ، ﺗﻌﺪاد ﺗﻨﻔﺲ و ﻣﻴﺰان اﺷﺒﺎع ﺑﻮدن ﻫﻤﻮﮔﻠﻮﺑﻴﻦ از ﺑﺎ اﺳﺘﻔﺎده از ﺳﻪ ادﻏﺎم داروﻳﻲ ﺑﻴﻬﻮﺷﻲ ﻣﺘﻔﺎوت ﻃﻲ اﻧﺪوﺳﻜﻮﭘﻲ ﻓﻮﻗﺎﻧﻲ دﺳﺘﮕﺎه ﮔﻮارش در ﺳﮕﻬﺎ ﺑﻮد. ﻃﺮح- ﻣﻄﺎﻟﻌﻪ ﺗﺠﺮﺑﻲ ﺣﻴﻮاﻧﺎت- 21 ﻗﻼده ﺳﮓ ﺳﺎﻟﻢ و ﺑﺎﻟﻎ از ﻧﮋاد ﻣﺨﻠﻮط ﺑﻪ ﺳﻪ ﮔﺮوه ﻣﺴﺎوي ﺗﻘﺴﻴﻢ ﺷﺪﻧﺪ. روش ﻛﺎر- ﺳﻪ ادﻏﺎم داروﻳﻲ ﻣﺘﻔﺎوت ﺑﺎ ﺗﺰرﻳﻖ داﺧﻞ ورﻳﺪي ﺟﻬﺖ ﺑﻴﻬﻮﺷﻲ در ﺳﮕﻬﺎ اﻋﻤﺎل ﺷﺪ. ادﻏﺎم داروﻳﻲ دﻳﺎزﭘﺎم و ﻛﺘﺎﻣﻴﻦ )0/2 و 6/5 ﻣﻴﻠﻲ ﮔﺮم ﺑﻪ ازاي ﻫﺮ ﻛﻴﻠﻮﮔﺮم وزن ﺑﺪن( در ﮔﺮوه KD، ادﻏﺎم داروﻳﻲ ﻣﻴﺪازوﻻم و ﻛﺘﺎﻣﻴﻦ )0/3 و 6/5 ﻣﻴﻠﻲﮔﺮم ﺑﻪ ازاي ﻫﺮ ﻛﻴﻠﻮﮔﺮم وزن ﺑﺪن( در ﮔﺮوه KMi و ادﻏﺎم داروﻳﻲ ﻣﺪﺗﻮﻣﻴﺪﻳﻦ و ﻛﺘﺎﻣﻴﻦ )0/4 و 6/5 ﻣﻴﻠﻲﮔﺮم ﺑﻪ ازاي ﻫﺮ ﻛﻴﻠﻮﮔﺮم وزن ﺑﺪن( در ﮔﺮوه KMed ﺗﺰرﻳﻖ ﺷﺪ. ﻛﻠﻴﻪ ﺑﻴﻤﺎران ﺗﺤﺖ اﻧﺪوﺳﻜﻮﭘﻲ ﻓﻮﻗﺎﻧﻲ دﺳﺘﮕﺎه ﮔﻮارش ﻗﺮار ﮔﺮﻓﺘﻨﺪ. ﺗﻌﺪاد ﺗﻨﻔﺲ و ﺿﺮﺑﺎن ﻗﻠﺐ در دﻗﻴﻘﻪ، درﺻﺪ اﺷﺒﺎع ﺑﻮدن ﻫﻤﻮﮔﻠﻮﺑﻴﻦ از اﻛﺴﻴﮋن، ﭘﻴﺶ از اﻟﻘﺎي ﺑﻴﻬﻮﺷﻲ، در زﻣﺎن ﻋﺒﻮر اﻧﺪوﺳﻜﻮپ از ﻣﺮي، ﻗﺎﻋﺪه ﻗﻠﺐ، ﻛﺎردﻳﺎ، درون ﻣﻌﺪه و ﻫﻨﮕﺎم ﺧﺮوج ﺛﺒﺖ ﺷﺪ. ﻫﻤﭽﻨﻴﻦ ﻣﺪت زﻣﺎن اﻟﻘﺎي ﺑﻴﻬﻮﺷﻲ و ﻃﻮل دوره ﺑﻴﻬﻮﺷﻲ در ﻛﻠﻴﻪ ﺳﮕﻬﺎ ﺛﺒﺖ ﺷﺪ. ﻧﺘﺎﻳﺞ- ﻣﺪت زﻣﺎن اﻟﻘﺎي ﺑﻴﻬﻮﺷﻲ و ﻃﻮل دوره ﺑﻴﻬﻮﺷﻲ ﺑﻪ ﺻﻮرت ﻣﻌﻨﻲداري در ﮔﺮوه KMi ﻧﺴﺒﺖ ﺑﻪ دو ﮔﺮوه دﻳﮕﺮ ﻛﻮﺗﺎﻫﺘﺮ ﺑﻮد P<0.05)(. ﻛﻠﻴﻪ ﺳﮕﻬﺎ دﭼﺎر درﺟﺎﺗﻲ از ﻫﻴﭙﻮﻛﺴﻲ ﺷﺪﻧﺪ. ﻫﺮ ﭼﻨﺪ درﺻﺪ اﺷﺒﺎع ﺑﻮدن ﻫﻤﻮﮔﻠﻮﺑﻴﻦ از اﻛﺴﻴﮋن در ﮔﺮوه KD و KMi ﺑﻪ ﺻﻮرت ﻣﻌﻨﻲ داري ﭘﺎﻳﻴﻦ ﺗﺮ ﺑﻮد)0.05<(P. ﻧﺘﻴﺠﻪﮔﻴﺮي و ﻛﺎرﺑﺮد ﺑﺎﻟﻴﻨﻲ- ﻫﺮ ﺳﻪ ادﻏﺎم داروﻳﻲ ﺗﺠﻮﻳﺰ ﺷﺪه ﻣﻮﺟﺐ ﺑﺮوز ﻫﻴﭙﻮﻛﺴﻲ ﺷﺪﻧﺪ. ﻫﺮ ﭼﻨﺪ ﺷﺪت ﻫﻴﭙﻮﻛﺴﻲ در ﮔﺮوﻫﻲ ﻛﻪ ادﻏﺎم ﻣﺪﺗﻮﻣﻴﺪﻳﻦ و ﻛﺘﺎﻣﻴﻦ ﺗﺠﻮﻳﺰ ﺷﺪه ﺑﻮد ﻛﻤﺘﺮ ﺑﻮد. درﻳﺎﻓﺖ اﻛﺴﻴﮋن ﻣﻜﻤﻞ از راه ﺑﻴﻨﻲ در ﻃﻲ اﻧﺪوﺳﻜﻮﭘﻲ ﻓﻮﻗﺎﻧﻲ دﺳﺘﮕﺎه ﮔﻮارش در ﺳﮕﻬﺎ در ﻣﻤﺎﻧﻌﺖ از ﺑﺮوز ﻫﻴﭙﻮﻛﺴﻲ ﺗﻮﺻﻴﻪ ﻣﻲ ﺷﻮد.
