Abstract :
Introduction: Oroantral communication is a significant maxillofacial problem, and it may be complicated with maxillary sinusitis, and different procedures were used for its management, through this study we used buccal pad of fat supported with buccal advancementmucoperiosteal flap. Aim of the Study: Is to evaluate repair of oroantral communication either it is recent or old usingbuccal pad of fat supported with buccal advancement mucoperiosteal flap. Patients and Methods: 16 patients presented with oroantral communication divided into two groups (in the 1st group 8 patients affected with acute communication while in the second group 8 patients affected with chronic fistula) all the patients treated using buccal pad of fat supported with buccal advancement flap and followed-up for any dehiscence or necrosis through 6 months. Results: 8 patients were presented with recent communication (within 24 hour) and other 8 patients presented with old communication (after 48 hour), 2 females represented (12.5%) and males were 14 (87.5%), the age range was from 20 to 45 years with the cause of oroantral communication was due to extraction of molar teeth in 10 patients (62.5%). However it was due to pathology in 6 cases (37.5%), the defect site was at the region of the upper 1st molar in 10 cases where in 6 cases the defect was at the region of upper 2nd molar, 4 (25%) cases was presented with maxillary sinusitis that was first managed then repaired by the flap design, there was eventful healing of the flap in all patients with little loss of buccal sulcus depth. Conculosion: Repair of the oroantral communication whether it is recent or old can be repaired successfully using buccal pad of fat supported with buccal advancement flap.
