Abstract :
InJapan,long-termhospitalizationofpatientswithschizophreniaisstillprevalent,eventhoughthefocusofpsychiatriccareis
shiftingfromhospitalstothecommunity.Difficultiesindischargeplanningoftenarisebecauseclinicalstaffmembers’functional
assessmentdiffersfromthatofpatients’self-assessment.Therefore,weattemptedtoidentifycharacteristicsrelatedtothese
perceptualdifferencestopromotethedevelopmentofmoreeffectiveapproachestowardthedischargeandsocietalreintegrationof
patientswithschizophreniaundergoingprolongedhospitalization.Forty-eightlong-terminpatients(23menand25womenwith
ameanageof60.72years)withschizophreniawereexaminedusingtheSchizophreniaCognitionRatingScaleJapaneseversion
(SCoRS-J),LifeSkillsProfile(LSP),andProfileofMoodStates-(POMS-)BriefForm.Differencesbetweenpatients’self-ratings
andclinicalstaffmembers’ratingsontheSCoRS-Jwereusedtodividepatientsintooverestimators,underestimators,andaccurate
raters.Thesegroupswerethencomparativelyanalyzed.Accordingly,overestimatorsdisplayedsignificantlyseverecognitive
dysfunctionontheSCoRS-Jobjectiveratings(p=:001)andsignificantlylessdifficultyontheSCoRS-Jsubjectiveratings(p=:002)
ascomparedtounderestimators.Overestimatorsalsoscoredsignificantlyloweronthecommunication(p=:012)and
responsibility(p=:021)LSPsubscalescomparedtounderestimators,andthetotalLSPscoreforoverestimatorswassignificantly
lowercomparedtoaccurateraters(p=:036)andunderestimators(p=:009).However,underestimatorsdisplayedsignificantly
higherconfusiononthetitularPOMSsubscalethandidoverestimators(p=:021).Thesefindingsindicatethat,amongthethree
groups,overestimatorsscoredlowestforobjectivelyratedfunctioning.Incontrast,underestimatorsattainedthehighest
functioning;however,theywerealsoconfused.Clinicalstaffshouldexaminehowpatients’self-perceptionsdeviatefromthe
perceptionsofstaffandimplementanappropriateapproachconsideringthepatientcharacteristicsrevealedfromtheresultsof
thisstudy
