  • Abstract
The Herlyn-Werner-Wunderlich syndrome (HWWS) is an uncommon Müllerian duct anomaly. A comprehensive approach to managing the diagnostic process, surgical intervention, and pregnancy planning is crucial. This report provides comprehensive management of a 26-year-old woman who experienced persistent menstrual pain and was directed to a fertility centre for further evaluation. The examination revealed the presence of Herlyn-Werner-Wunderlich syndrome, accompanied by Hematocolpos, focal adenomyosis, a large endometrioma, and rectovaginal endometriosis. Collaborative operative management with a urogynecologist included laparoscopic procedures to address endometriosis and excision of the vaginal septum under laparoscopic guidance. Following surgery, the patient underwent regular follow-up care at the fertility clinic. After one year, she participated in a pregnancy program and conceived naturally. Due to the obstetric indications, she underwent a cesarean section delivery at 37 weeks of gestation. In general, this case exemplifies the entire journey from diagnosis, through preoperative and surgical care, involvement in a prenatal program, and oversight during pregnancy, ultimately resulting in a delivery with favorable and successful outcomes.
