Abstract :
Purpoe
To decribe the clinical feature of a diorder characterized by chronic multifocal retinal infiltrate and uveiti in individual with human immunodeficiency viru (HIV) dieae.
Method
We reviewed the medical record of HIV-infected patient with multifocal retinal infiltrate of unknown caue een by invetigator at four intitution. The following data were collected: demographic characteritic, preenting ign and ymptom, laboratory tet reult, and coure of dieae.
Reult
We identified 26 HIV-infected patient (50 involved eye) with thi yndrome. Median CD4+ T-lymphocyte count at preentation wa 272 per μl (range, 7 to 2,118 per μl). The mot common preenting ymptom wa floater. Median viual acuity of involved eye at preentation wa 20/20 (range, 20/15 to 20/100) and remained table (median, 20/20; range, 20/15 to 20/70) after a median follow-up period of 9 month (range, 0 to 110 month). Typical retinal leion were gray-white or yellow, irregular in hape, and le than 200 (Am in greatet dimenion. All were located in the midperiphery or anterior retina and enlarged lowly or remained tatic in ize. Mild to moderate anterior chamber or vitreou humor inflammatory cell were preent in 47 of 50 eye (26 of 26 patient). Retinal leion poibly reponded to zidovudine but not to acyclovir or ganciclovir. Anterior chamber and vitreou humor inflammatory reaction reponded to topical or periocular injection of corticoteroid.
Concluion
Uveiti with chronic multifocal retinal infiltrate i a ditinct clinical entity of unknown caue that occur in HIV-infected patient. Retinal leion may repond to antiretroviral therapy. Viual prognoi i good.
