Abstract :
PURPOE: To determine whether quantitative difference in ytemic hypercoagulable tate could be identified among patient with retinal vein occluion at variou ite of occluion.
METHOD: The value of thrombin–antithrombin III complex wa determined in 57 patient with retinal vein occluion within 1 month after the ubjective onet of retinal vein occluion and in 15 age-matched normal control.
REULT: Level of log thrombin–antithrombin III complex were ignificantly higher in the patient with proximal retinal vein occluion in which the occluion ite i at the optic dic (mean ± D, 0.493 ± 0.389) than in thoe with dital retinal vein occluion in which the occluion ite i away from the optic dic (0.312 ± 0.150, P = .025) and in the normal control (0.294 ± 0.151, P = .020). There wa no ignificant difference between the dital retinal vein occluion and the normal control (P = .720). More patient with proximal retinal vein occluion howed elevated thrombin–antithrombin III complex value more than 3.9 ng/ml than thoe with dital retinal vein occluion (8/29 v 1/28, P = .025). Of nine patient howing an initial value of thrombin–antithrombin III complex of more than 3.9 ng/ml, repeated meaurement were obtained in eight patient, who howed reduced value of thrombin–antithrombin III complex in the normal range in everal month.
CONCLUION: A ytemic hypercoagulable tate, which could be demontrated with the elevation of thrombin–antithrombin III complex value, may contribute more to the development of retinal vein occluion with thrombu at or near the trunk of the central retinal vein than thoe with thrombu at branch vein away from the optic dic in the retina.
