PURPOE: To report a cae of iolated inferior rectu paly econdary to a metatai to the oculomotor nucleu. METHOD: Cae report. A 41-year-old woman with a hitory of breat cancer preented with acute onet of left hypotropia and exotropia. REULT: Forced generation teting confirmed weakne of the right inferior rectu mucle that wa not revered by intravenou edrophonium infuion. Magnetic reonance imaging dicloed numerou metatatic leion to the cerebral hemiphere and braintem. One leion in the right midbrain wa adjacent to the cerebral aqueduct in the right oculomotor nucleu. CONCLUION: Metatai to the oculomotor nucleu i a rare caue of iolated inferior rectu paly; however, thi entity hould be conidered in the differential diagnoi of an iolated inferior rectu paly becaue of the life-threatening conequence of a braintem leion.
