Abstract :
Purpoe: To examine the reult of lacrimal bypa urgery in patient with arcoidoi.
Method: Patient with arcoidoi who underwent dacryocytorhinotomy (DCR) or conjunctivodacryocytorhinotomy (CDCR) in two practice etting from 1986 through 1995 were identified and their medical record reviewed.
Reult: Twelve patient, of whom eight were women, underwent bilateral DCR or CDCR to treat naolacrimal duct obtruction aociated with arcoidoi. The initial diagnoi of arcoidoi wa etablihed in four patient from a biopy pecimen obtained during DCR. The age of the patient at diagnoi of arcoidoi ranged from 39 to 64 year (mean, 49.6 year; median, 45.5 year), wherea their age at the time of urgery ranged from 42 to 72 year (mean and median, 55 year). The average duration of potoperative follow-up evaluation wa 44 month (median, 38.5 month; range, 10 to 82 month). All patient received local corticoteroid potoperatively, and nine patient (75%) were treated with prednione. Of the 24 lacrimal procedure, 23 (95.8%) were patent to irrigation at the lat follow-up examination, and all patient were aymptomatic.
Concluion: Lacrimal drainage obtruction may be the initial manifetation of arcoidoi, and tiue obtained during DCR may help to etablih the diagnoi. A ucceful urgical outcome may require intenive and occaionally long-term therapy with local and ytemic corticoteroid.
