Abstract :
PURPOE: To decribe the ocular and ytemic feature of children with cytomegaloviru retiniti and their dieae outcome.
METHOD: Review of all cae of cytomegaloviru retiniti diagnoed or treated at a tertiary care pediatric hopital during a 10-year period.
REULT: Nine immunocompromied children younger than 16 year were diagnoed a having cytomegaloviru retiniti. The underlying caue of immunocompromie were evere combined immunodeficiency yndrome (n = 2), evere combined immunodeficiency yndrome after bone marrow tranplantation (n = 1), acquired immunodeficiency yndrome (AID) (n = 2), AID and previou bone marrow tranplantation for leukemia (n = 1), immunouppreive therapy after renal tranplantation (n = 1), chemotherapy for leukemia (n = 1), and congenital cytomegaloviru infection (n = 1). Five children (56%) had ymptomatic extraocular cytomegaloviru infection. Only two children reported viual ymptom with cytomegaloviru retiniti at initial examination. Cytomegaloviru retiniti wa bilateral in eight children (89%) and involved the poterior pole in at leat one eye of all nine children. Four children (44%) died within 10 month of being diagnoed with cytomegaloviru retiniti. The remaining five children were alive, with follow-up ranging from 14 to 70 month. ucceful bone marrow tranplantation in one child and dicontinuation of immunouppreive medication in two children improved ytemic immune function and permitted dicontinuation of anticytomegaloviral therapy.
CONCLUION: Pediatric cytomegaloviru retiniti i often aymptomatic and bilateral and involve the poterior pole at initial examination. Recovery of ytemic immune function may occur in ome children. Evaluation of children at rik and prompt treatment of cytomegaloviru retiniti are important to prevent long-term viual morbidity.
