Abstract :
PURPOE: To evaluate the retinal tructure before and after vitrectomy for diabetic macular edema and to ae the correlation between thickne of neuroenory retina and bet-corrected viual acuity. METHOD: Tomographic feature of 13 eye (nine patient) with diabetic macular edema were propectively evaluated with optical coherence tomography before and after vitrectomy. The foveal thickne (the ditance between the inner retinal urface and the retinal pigment epithelium) and the retinal thickne (thickne of neuroenory retina) were meaured by optical coherence tomography preoperatively and potoperatively. The correlation of the bet-corrected viual acuity with foveal and retinal thickne wa determined. REULT: All 13 eye had retinal welling with low intraretinal reflectivity. In addition to retinal welling, there were cytoid pace in five (38%) of 13 eye, a erou retinal detachment in three (23%), and both cytoid pace and erou detachment in three (23%). ix month potoperatively, the mean foveal thickne ignificantly decreaed from 630 ± 170 to 350 ± 120 μm (P< .01, paired t tet) and the mean thickne of neuroenory retina decreaed from 540 ± 160 to 320 ± 140 μm (P< .01, paired t tet). A erou retinal detachment occurred traniently in 3 eye. Compared with the preoperative level, the potoperative bet-corrected viual acuity level improved by more than 2 line in five of the 13 eye (38%), remained the ame in even eye (54%), and decreaed in one eye (8%). The potoperative thickne of neuroenory retina at the fovea and bet-corrected viual acuity level at the ixth potoperative month had a trong negative correlation (correlation coefficient, −0.76; P< .01, pearman rank tet). CONCLUION: Vitrectomy wa generally effective in treatment of diabetic macular edema. Optical coherence tomography demontrated the intraretinal change of macular edema and the proce of edema aborption. During the proce of macular edema aborption, intraretinal fluid appeared to move into the ubretinal pace in ome cae. Bet-corrected viual acuity improvement wa greater in eye with le preoperative increae in thickne of neuroenory retina.
