Abstract :
PURPOE: To evaluate the tructure of the anterior egment after congenital cataract urgery with ultraound biomicrocopy.
METHOD: Ultraound biomicrocopy wa performed on 28 eye of 15 patient after congenital cataract urgery (age range, 8–27 year; mean, 13.1 ± 1.1 year), of which 26 eye had par plana lenectomy and two eye had phacoemulification and apiration, and on 21 eye of 11 phakic control ubject who were healthy (age range, 6–17 year; mean, 11.5 ± 1.1 year). The angle-opening ditance at 500 μm wa meaured on a line perpendicular to the trabecular mehwork 500 μm from the cleral pur, and the trabecular-iri angle wa meaured.
REULT: Angle-opening ditance at 500 μm in the congenital cataract group ranged from 174 to 920 μm (mean, 424 ± 28 μm), and in the control group it ranged from 260 to 1,119 μm (mean, 643 ± 57 μm), a difference that wa ignificantly different (P = .0004, tudent t tet). The trabecular-iri angle in the congenital cataract group ranged from 30.4 to 66.9 degree (mean, 41.9 ± 1.6 degree) and in the control group ranged from 32.1 to 69.8 degree (mean, 44.9 ± 2.5 degree), a difference that wa not ignificantly different (P = .28, tudent t tet). Thi mean that there i a tendency for high inertion of the iri in the congenital cataract group, and intraocular preure wa elevated after urgery (including glaucoma and ocular hypertenion) in 10 (47.6%) of 21 eye with high inertion. The par plicata after congenital cataract urgery wa ignificantly flatter than that of control ubject (P = .0057, tudent t tet).
CONCLUION: Ultraound biomicrocopy i a ueful method for evaluating the anterior egment of eye with congenital cataract, and complication after cataract urgery can be caued by diorganization of the anterior egment. Patient with abnormalitie of the anterior egment hould be followed up cloely for the development of glaucoma.
