Abstract :
PURPOE: To report the long-term outcome of intraocular preure after laer peripheral iridotomy in Aian eye with acute primary angle-cloure.
METHOD: Retropective tudy of 111 eye of 96 conecutive patient with acute primary angle-cloure, preenting at the National Univerity Hopital, ingapore, from 1990 to 1994. The preenting feature of the affected eye and the treatment intituted were recorded. The ubequent long-term intraocular preure outcome wa analyzed. An increae in intraocular preure on follow-up wa defined a increae in intraocular preure greater than 21 mm Hg and requiring treatment by medication or urgery.
REULT: The mean follow-up period wa 50.3 month (range, 9 to 107 month). The mean preenting intraocular preure wa 52.8 mm Hg (range, 28 to 80 mm Hg). One hundred ten eye were treated with laer peripheral iridotomy, with reolution of the acute epiode and intraocular preure le than 21 mm Hg in all eye after laer peripheral iridotomy. Of thee, only 46 eye (41.8%) were uccefully treated with laer peripheral iridotomy alone in the long term. ixty-four eye (58.1%) developed an increae in intraocular preure (requiring treatment) on follow-up, of which 49 eye developed an increae in intraocular preure within the firt 6 month after acute primary angle-cloure. Thirty-ix eye (32.7%) eventually underwent trabeculectomy becaue of uncontrolled intraocular preure depite laer and medical therapy.
CONCLUION: In thi tudy of Aian eye, a high proportion (58.1%) of eye with acute primary angle-cloure developed an increae in intraocular preure on long-term follow-up after reolution of the acute attack, depite the preence of a patent laer peripheral iridotomy. Thee reult ugget a racial difference in the outcome of laer peripheral iridotomy after acute primary angle-cloure in Aian, compared with Caucaian. Becaue a majority of eye that develop an increae in intraocular preure do o within the firt 6 month of preentation, cloe monitoring of intraocular preure i advied in the follow-up of patient with acute primary angle-cloure
