New corneal finding in human T-cell lymphotrophic viru type 1 infection

PURPOE: Human T-cell lymphotrophic viru type 1 i a RNA retroviru that primarily affect CD4+ T-cell. Human T-cell lymphotrophic viru type 1 infection i the etablihed caue of adult T-cell leukemia/lymphoma, an aggreive malignancy of CD4+ T-cell, and two nonneoplatic condition: human T-cell lymphotrophic viru type 1–aociated myelopathy/tropical patic paraparei and human T-cell lymphotrophic viru type 1 uveiti. Other reported ophthalmic manifetation of human T-cell lymphotrophic viru type 1 infection include lymphomatou and leukemic infiltrate in the eye and ocular adnexa in patient with adult T-cell leukemia/lymphoma, retinal pigmentary degeneration, and neuro-ophthalmic diorder in patient with human T-cell lymphotrophic viru type 1–aociated myelopathy/tropical patic paraparei and keratoconjunctiviti icca, epicleriti, and clerouveiti in aymptomatic human T-cell lymphotrophic viru type 1 carrier. Thi report decribe the ocular finding in three Jamaican patient with human T-cell lymphotrophic viru type 1 infection and adult T-cell leukemia/lymphoma. METHOD: The clinical record of three patient with human T-cell lymphotrophic viru type 1 infection and adult T-cell leukemia/lymphoma examined at the National Eye Intitute were reviewed. Each patient had one or more complete ophthalmic evaluation. REULT: All three patient had corneal abnormalitie, including corneal haze and central opacitie with thinning; bilateral immunoprotein keratopathy; and peripheral corneal thinning, carring, and neovacularization. All three patient had elevated erum immunoglobulin level. CONCLUION: We believe that the novel corneal finding in thee patient are mot likely a conequence of the hypergammaglobulinemia induced by the human T-cell lymphotrophic viru type 1 infection or the T-cell malignancy.
