Abstract :
PURPOE: To evaluate the effect of attempted eyelid cloure on intraocular preure meaurement. METHOD: Normal ubject underwent intraocular preure meaurement in both eye uing Goldmann applanation tonometry and Tono-pen XL (Mentor, Inc, Norwell, Maachuett) by the ame examiner holding the eyelid open, both with and without the ubject imultaneouly attempting forced eyelid cloure. ubject were eated during all meaurement and waited 5 minute between meaurement with each intrument; the order of meaurement wa randomized. REULT: Thirty eye of 15 ubject (ix men, nine women) were enrolled. Mean age wa 30.5 ± 5.2 year (range, 24 to 40 year). With Goldmann applanation tonometry, intraocular preure increaed in both eye with attempted eyelid cloure by a mean of 1.5 ± 2.0 mm Hg (P = .0002, paired t tet; range, −2 to 8 mm Hg). With the Tono-pen XL, intraocular preure alo increaed in both eye with attempted eyelid cloure by a mean of 1.9 ± 2.7 mm Hg (P = .0002, paired t tet; range, −2 to 9 mm Hg). Tono-pen XL mean intraocular preure value in both eye (14.4 ± 2.3 mm Hg) conitently overetimated thoe of Goldmann applanation tonometry (13.0 ± 2.2 mm Hg) by a mean of 1.4 ± 2.3 mm Hg. CONCLUION: Attempted forced eyelid cloure i a common and tatitically ignificant ource of error in routine outpatient meaurement of intraocular preure and could influence clinical management of glaucoma.
