Abstract
PURPOE: To invetigate the long-term incidence of poterior capular opacification after phacoemulification compared with phacotrabeculectomy with or without adjunctive ubconjunctival mitomycin C.
METHOD: Thi wa a retropectively conducted long-term, obervational, cae-control tudy. One hundred eye of 100 cataract patient who underwent phacoemulification and poterior chamber intraocular len implantation and 100 eye of 100 primary open-angle glaucoma patient with cataract that underwent phacotrabeculectomy and poterior chamber intraocular len implantation, matched with repect to age, intraocular len type, prevalence of diabete mellitu, and length of follow-up. The main outcome meaure wa the rate of clinically ignificant poterior capular opacification a determined by lit-lamp biomicrocopy and neceity to perform neodynium:yttrium aluminum garnet (Nd:YAG) capulotomy and a calculated by Kaplan-Meier urvival analyi. Potoperative viual acuity and maintenance of intraocular preure control were alo meaured.
REULT: There wa no ignificant difference in the rate of poterior capular opacification requiring Nd:YAG capulotomy between the phacoemulification and phacotrabeculectomy group (P = .77). However, a ignificant difference in the rate of poterior capular opacification wa found between thoe patient without diabete mellitu and thoe with a preoperative diagnoi of diabete mellitu (P = .016). Alo, urvival analyi comparing ue of mitomycin C with no ue of mitomycin C in the phacotrabeculectomy group howed a higher urvival in the mitomycin C ubgroup (P = .03).
CONCLUION: There wa no ignificant difference in long-term poterior capular opacification between phacoemulification and phacotrabeculectomy in the tudy population. Intraoperative, adjunctive ue of mitomycin C in the phacotrabeculectomy group and the preence of diabete mellitu in the overall patient were beneficial (protective) factor againt poterior capular opacification.
