Abstract :
PURPOE: To identify clinical and microbiologic factor influencing the prevalence and viual outcome in endophthalmiti after deadly-weapon-related open-globe injurie.
METHOD: In a retropective tudy of 228 eye of 212 patient with deadly-weapon-related open-globe injurie, clinical and microbiologic factor influencing the prevalence and viual outcome in endophthalmiti were analyzed in detail.
REULT: Twenty-nine eye enucleated for irreparable damage at preentation were excluded. Nineteen eye (18 patient) of 199 (186 patient) were aociated with culture-proven endophthalmiti. Preence of grade 4 injury, a variable of Ocular Trauma Claification ytem, and len diruption at preentation were the ignificant rik factor for development of endophthalmiti (P = .001; odd ratio = 15.9 [2.0 to 122.1]), and ([P < .001; odd ratio = 17.7 [2.3 to 136.3]), repectively. Favorable outcome (viual acuity of 5/200 or better) wa achieved in only four eye (21%). Five eye (26.3%) were phthiical, and five eye (26.3%) were enucleated or evicerated. All eye were under the coverage of prophylactic intravenou antibiotic when culture poitivity wa obtained. Except two eye with Acinetobacterp., all were infected by gram-poitive micro-organim (89.5%), the mot common of which (42%) wa taphylococcu epidermidi. None of the clinical characteritic and treatment modalitie affected final viual outcome. Favorable outcome wa aociated with infection by the le virulent micro-organim . epidermidi (P = .018; odd ratio = 0.50 [0.25 to 1.00]).
CONCLUION: Endophthalmiti after deadly-weapon–related open-globe injurie ha a dimal viual prognoi. everity of injury and len diruption are ignificant rik factor for development of endophthalmiti. Infection with a le virulent microbe i the only factor aociated with favorable outcome
