Abstract :
PURPOE: To decribe the characteritic and evolution of macular retinochii in high myopia oberved by optical coherence tomography (OCT).
DEIGN: A conecutive, retropective, obervational cae erie.
METHOD: Twenty-one highly myopic eye (mean refractive error, −15.2, range −6 to −25) of 17 patient preenting with the unuual feature of macular thickening without a macular hole and aociated with a poterior taphyloma were examined by biomicrocopy and OCT. Ten patient (13 eye) were followed up for 12 month or more.
REULT: On biomicrocopy, the macula of all 21 eye had a microcytic appearance without macular hole. In all eye, OCT howed that retinal thickening wa mainly due to an extenive hyporeflective pace plitting the neuroretina into a thick inner layer and a thin outer layer. We called thi condition outer retinochii. In ix cae, inner plitting, termed inner retinochii, wa alo preent. The macular profile exhibited a foveal cyt in 10 eye, a lamellar hole in ix, and a foveal detachment in ix. In four of the 21 eye, a hyperreflective preretinal tructure reembling the poterior hyaloid wa tretched over the retinochii, cauing foveal traction. Two of thee four eye ubequently evolved into a full-thickne macular hole.
CONCLUION: Macular retinochii i not uncommon in highly myopic eye with taphyloma and i better characterized by OCT than by biomicrocopy. Intraretinal plitting occur in both the outer and inner layer of the retina, leading to the formation of cytoid pace. In mot cae, the condition i fairly table in term of viual acuity and retinal thickne and change occur lowly over time. However, a macular hole may occur when the retinochii i aociated with tangential traction of the poterior hyaloid
