Abstract :
Purpoe To define the chorioretinal leion created unintentionally during graping the internal limiting membrane (ILM) with end-gripping forcep. Deign Interventional cae erie. Method A propective review of a conecutive erie of 15 patient (15 eye) who underwent macular hole repair wa performed. All patient underwent a three-port par plana vitrectomy with ILM peeling. Fundu photograph, fluorecein angiography, and indocyanine green angiography were performed in all patient before and after urgery. Reult In all eye, mall punctate barely een chorioretinal leion in the macular region were detected after urgery. Thee punctate leion correponded to the area where ILM wa graped with forcep. The leion ranged from 100 to 400 μm in diameter. Their number ranged between 8 and 15 (mean, 12.6). In the early phae of fluorecein angiography, the leion appeared hypofluorecent; in the late phae, there wa light taining of the margin of the leion extending from the adjacent choriocapillari, cauing inditinct border. In the early phae of indocyanine green angiogram, the leion were hypofluorecent and in ome larger leion choroidal veel were viible due to the thinning and atrophy of the retinal pigment epithelium and choriocapillari. In the late phae, the homogenou hypofluorecent area had well-delineated margin and were uually round or oval. Thi angiographic finding wa independent of the age of the leion, and neither enlargement of the leion nor development of choroidal neovacularization were noted during follow-up. Concluion Iatrogenic punctate chorioretinopathy i the chorioretinal leion created unintentionally during graping the ILM with end-gripping forcep. It mut be reported a one of the complication of ILM peeling. Thee mall punctate leion did not appear to affect the urgical outcome. However, the leion hould be followed to detect any long-term complication uch a choroidal neovacularization.
