Abstract :
Purpoe To review management trategie for treatment of anterior chamber epithelial cyt. Deign Retropective review of conecutive interventional cae erie. Method Chart of patient treated for epithelial ingrowth over a 10-year period by a ingle urgeon were reviewed. Cae of anterior chamber epithelial cyt were identified and recorded, including detail of ocular hitory, preoperative and potoperative acuity, intraocular preure (IOP), and ocular examination, type of urgical intervention, and detail of further procedure performed. Reult even eye with epithelial cyt were identified. Patient age ranged from 1.5 to 53 year at preentation. Four patient were children. In four eye, cyt were econdary to trauma, one cae wa preumably congenital, one cae developed after corneal perforation in an eye with Terrien’ marginal degeneration, and one cae developed after penetrating keratoplaty (PK). Three eye were treated with vitrectomy, en bloc reection of the cyt and aociated tiue, fluid–air exchange and cryotherapy. The lat four eye were treated with a new conervative trategy of cyt apiration (three cae) or local exciion (one keratin “pearl” cyt), and endolaer photocoagulation of the collaped cyt wall/bae. All epithelial tiue wa uccefully eradicated by clinical criteria; one cae required repeat exciion (follow-up, 9 to 78 month, mean 45). Two eye required later urgery for elevated IOP, two for cataract extraction and one for repeat PK. Final viual acuity ranged from 20/20 to hand motion, depending on aociated ocular damage. Bet-corrected viual reult were obtained in the more conervatively managed eye. Concluion Anterior chamber epithelial cyt can be managed conervatively in elected cae with good reult. Thi trategy may be particularly ueful in children’ eye, where preervation of the len, iri, and other tructure may facilitate amblyopia management.
