  • Abstract
    Purpoe To identify factor aociated with evere uveiti at diagnoi of eye dieae in children with juvenile idiopathic arthriti (JIA) who were oberved in a creening program, and to identify temporal change in finding aociated with creening during the period 1986 to 2000. Deign Cae control tudy. Method Uveiti wa defined a being evere at diagnoi if there were aociated ynechiae, cataract, or retinal edema. Clinical factor for all patient undergoing creening for JIA-aociated uveiti from 1986 to 2000 at one intitution were tudied retropectively. Children with evere uveiti at diagnoi (cae) were compared with thoe with mild uveiti at diagnoi and thoe who never developed uveiti during follow-up in the creening program (control). Reult evere uveiti wa preent at diagnoi in 22 (5.4%) of 409 patient. Male patient were more likely to have evere uveiti at diagnoi (odd ratio [OR] 3.7 [95% confidence interval 1.3 to 10.7], P = .014). A longer interval from the onet of arthriti ymptom to the diagnoi of uveiti wa aociated with a reduced rik of evere uveiti at diagnoi (OR 0.95 [0.91 to 1.0], P = .044). There wa no difference in the frequency of evere uveiti at diagnoi between two creening period: 1986 to 1993 (before American creening guideline were publihed) and 1994 to 2000. Concluion Male children are more likely than female children to have evere uveiti at diagnoi. The proportion of children with evere uveiti at diagnoi ha not changed ince current creening guideline have been widely publicized, uggeting the need for refinement in creening procedure to target high-rik children with increaed urveillance
