Abstract :
Purpoe
Breat cancer i an increaingly important health problem in women and i the mot common tumor to metataize to the uvea. Thi tudy wa deigned to evaluate the clinical feature, management, and prognoi of patient with uveal metatai from breat cancer.
Deign
Retropective interventional cae erie.
Method
We retropectively reviewed 264 conecutive patient with uveal metatai from breat cancer. We aeed the clinical feature of the patient and tumor at the time of preentation, management, and prognoi. Kaplan-Meier urvival etimate were ued to analyze the probability of death a a function of time.
Reult
Uveal metatai wa the initial manifetation of breat cancer in even patient (3%) and the firt ytemic metatatic ite of previouly diagnoed breat cancer in 43 (16%). Aociated with uveal metatai, optic dik metatai wa found in 13 patient (5%), eyelid metatai in one patient (1%), and conjunctival and orbital in one patient (<1%). Of 264 patient with uveal metatai, 225 (85%) had choroidal metatai, eight (3%) iri metatai, two (<1%) ciliary body metatai, and 29 (11%) had metatai in multiple uveal ite. In the 264 patient with uveal metatai, the mot common ymptom wa blurred viion in 197 patient (88%), floater in 15 (5%), photopia in 12 (5%), and 19 (7%) were aymptomatic. The uveal metatae were bilateral in 99 patient (38%) and unilateral in 165 (62%). In 55 (56%) of the 99 bilateral cae, a uveal metatai wa found in the aymptomatic fellow eye during follow-up examination. External beam radiotherapy wa ued in 137 patient with uveal metatai (52%), providing tumor control in 116 patient (85%) at a mean follow-up of 21 month. Uing Kaplan-Meier etimate, urvival rate of all patient with uveal metatai from breat cancer wa 65% at 1-year, 34% at 3-year, and 24% at 5-year follow-up.
Concluion
Patient with uveal metatai from breat cancer preented to ophthalmologit with viual ymptom in 93% of cae. However, aymptomatic metatae were commonly detected in the fellow eye. Local ocular tumor control wa excellent with current therapie. However, ytemic prognoi for all patient, including thoe who had been treated with different management option, wa poor with urvival rate of 65% at 1-year and 24% at 5-year follow-up.
