Abstract :
Purpoe
To report the clinical feature, cauative organim, management, and viual acuity outcome of eight eye of eight patient who developed acute potoperative endophthalmiti following intravitreal injection of triamcinolone acetonide (IVTA).
Deign
Retropective, multicenter, interventional, cae erie.
Method
A retropective, interventional, cae erie of all patient with acute potoperative endophthalmiti following IVTA at even academic clinical center between March 2001 and July 2002.
Reult
A total of 922 IVTA were performed. Eight eye of eight patient with acute potoperative endophthalmiti were identified in the 6 week following IVTA for an incidence of 0.87% (95% confidence interval of 0.38% to 1.70%). The median time to preentation wa 7.5 day (range, 1–15 day) after IVTA. The mot common clinical finding were iriti (n = 8), vitriti (n = 8), hypopyon (n = 8), pain (n = 7), red eye (n = 6), and decreaed viion (n = 5). The median preenting viual acuity wa 20/1127 (range, 20/60 to light perception). Initial treatment conited of vitreou tap and injection of antibiotic (n = 6) or par plana vitrectomy and injection of intravitreal antibiotic (n = 2). Intraocular culture yielded identification in even patient. One demontrated intracellular gram-poitive cocci in chain with numerou polymorphonuclear cell on gram tain. The median potinfection viion wa 20/400 (range, 20/40 to no light perception). Three patient ended up with no light perception viual acuity, including enucleation (n = 1) and phthii (n = 1).
Concluion
Acute potoperative endophthalmiti following IVTA occur rapidly and can reult in evere lo of viion.
