  • Abstract
    Purpoe To evaluate the effect of ubfoveal choroidal neovacularization econdary to age-related macular degeneration on functional parameter obtained by canning laer ophthalmocope microperimetry. Deign Retropective obervational cae erie and cro-ectional tudy. Method At the Doheny Retina Intitute and Wilmer Eye Intitute a conecutive erie of 179 eye of 175 patient with ubfoveal choroidal neovacularization econdary to age-related macular degeneration wa tudied. The onet of viual ymptom, bet-corrected viual acuity, fluorecein angiography, evaluation of fundu microperimetry and fixation pattern uing the Rodentock canning laer ophthalmocope were obtained for each patient. The main outcome meaure were central retinal enitivity and fixation pattern (fixation location and fixation tability) in eye with ubfoveal choroidal neovacularization and their relationhip to the length of dieae, type and characteritic of choroidal neovacularization, and viual acuity. Reult Of 179 eye, 135 (75%) had central fixation, 27 (15%) had poor central fixation, and 17 (9%) had predominantly eccentric fixation. eventy-ix eye (42%) had table fixation, 70 eye (39%) had relatively untable fixation, and 33 eye (18%) had untable fixation. In 50 eye (28%) a dene central cotoma wa noted. Eighty-nine of 100 eye (89%) with length of ymptom of le than 3 month had predominantly central fixation and 58 (58%) had table fixation; 14 of 34 eye (41%) with length of ymptom of more than 6 month had predominantly central fixation, and 5 eye (15%) had table fixation. In 15 eye of patient who elected not to receive treatment, ucceive canning laer ophthalmocope microperimetry were obtained over time (follow-up of 18 month after onet of ymptom). Three month or le after the onet of ymptom, 13 eye (87.7%) had predominantly central fixation and 9 eye (60%) had table fixation. More than 3 month and 6 month or le after the onet of ymptom, 10 eye (66.7%) had predominantly central fixation and 7 eye (46.7%) had table fixation. Thi trend wa further demontrated in eye more than 6 month after the onet of ymptom. Concluion We conclude that the equence of event leading to viual function deterioration appear to involve an initial mild decreae in central retinal enitivity and viual acuity followed by progreive fixation intability and, ultimately, development of an abolute central cotoma with totally eccentric fixation. Increaed length of dieae i aociated with wore fixation pattern and retinal enitivity deterioration a aeed by canning laer ophthalmocope microperimetry. A better undertanding of the characteritic of viual lo aeed by fixation pattern evaluation and microperimetry in age-related macular degeneration may help optimize timing, patient election, and treatment option in eye with thi condition.
