Abstract :
Purpoe To compare aqueou humor concentration of levofloxacin v ciprofloxacin when ued a prophylactic medication before phacoemulification. Deign Patient (n = 93) were randomly aigned to receive either 0.5% levofloxacin (Quixin) or 0.3% ciprofloxacin (Ciloxan) uing one of the following doing regimen: (A) 1 to 2 drop four time a day for 2 day preoperatively; (B) 5 doe (1 to 2 drop) delivered every 10 minute in the hour immediately preceding urgery; or (C) the combination of A and B. Method Aqueou ample (0.1 ml) were obtained immediately before urgery, and drug concentration were meaured uing high performance liquid chromatography/ma pectrometry. Reult The mean concentration of levofloxacin in the aqueou humor wa ignificantly greater than that of ciprofloxacin in all treatment group (P < .001): 284.8 v 67.4 μg/ml (regimen A); 1,135.6 v 185.6 μg/ml (regimen B); and 1,618.6 v 241.5 (regimen C). Doing regimen B delivered ignificantly more drug to the aqueou humor than regimen A for both levofloxacin (P ≤ .001) and ciprofloxacin (P = .004). Doing regimen C delivered ignificantly more drug to the aqueou humor than regimen B for levofloxacin (P = .05) but not for ciprofloxacin (P = .384). Concluion Although the concentration of active drug in levofloxacin i approximately 1.7-fold higher than that in ciprofloxacin, the aqueou concentration of levofloxacin after topical adminitration wa four to even time greater than ciprofloxacin; thee difference were tatitically ignificant. With doing regimen B and C, levofloxacin concentration in the aqueou humor were above the MIC90 for mot common ocular pathogen, including taphylococcu and treptococcu pecie. Ciprofloxacin did not reach uch concentration in any treatment group.
