Abstract :
Purpoe To compare grading of cytomegaloviru retiniti progreion by two different fundu photograph reading center (FPRC). Deign Patient with AID followed in an ophthalmology ervice were enrolled in a propective epidemiologic tudy of the prevalence and incidence of reitant cytomegaloviru in vitro. Method We compared maked replicate grading by two different FPRC of monthly fundu photograph for retiniti progreion (onet of a eparate new leion or expanion of an exiting leion by ≥one-half dik diameter). Reult For 77 patient, Kaplan-Meier plot of progreion over time were imilar between FPRC (median time to progreion, 65 v 69 day). Agreement between FPRC wa 51% (kappa [κ] = .37, “fair”) on exact viit of progreion (28 patient) or on abence of progreion through follow-up (11 patient) and 62% (κ = .38) on progreion viit ± 1 month. Eight of 12 patient with progreion graded a more than 1 month apart were only 2 month apart. Conidering each monthly viit a a choice point, overall agreement on progreion wa 78% (κ = .55, “moderate”). Baeline evaluation of retiniti howed 95% agreement on preence/abence and a concordance correlation coefficient of .75 for extent in combined zone 1 and 2. Rate of retinal lo over follow-up were etimated a 2.8%/month v 2.0%/month (P = .015). Concluion By adopting imilar protocol and procedure, different FPRC can achieve good agreement on preence and extent of cytomegaloviru retiniti. Further effort to harmonize evaluation through ongoing comparion of grading would likely improve agreement on retiniti progreion.
