  • Abstract
    Purpoe To review the evidence that exit concerning the pathogenei of leion in late age-related macular dieae (AMD). Deign Review of the literature. Method A review of both experimental evidence and clinical obervation that addre thee problem. Reult There i good evidence that choroidal neovacularization (CNV) i due to a change in the balance of growth factor derived from the retinal pigment epithelial baolateral plama membrane domain (retinal pigment epithelium). Retinal angiomatou proliferation may alo have a imilar pathogenei involving the apical domain. Detachment of the retinal pigment epithelium i likely to be a conequence of increaed reitance of the Bruch membrane to water flow due to depoition of lipid. Geographic atrophy i preceded by accumulation of autofluorecent material in the retinal pigment epithelium and poible caual relationhip between the two have been demontrated. Concluion There i increaing undertanding concerning the equence of event that lead to thoe leion cauing lo of central viion in AMD. Therapeutic approache that addre the underlying mechanim are more likely to ucceed than current treatment option. uch an approach ha already been initiated in the management of choroidal neovacularization.
