Vogt-Koyanagi-Harada yndrome and keratoconjunctiviti icca

Purpoe To dicover a relationhip between Vogt-Koyanagi-Harada (VKH) yndrome and jögren yndrome. Deign Obervational cae-control tudy. Method ixteen Italian patient with VKH yndrome and 16 control with diffue uveiti underwent tear analyi and rheumatologic evaluation. Biopy pecimen of the minor alivary gland were obtained or alivary gland cintigraphy wa performed in patient with xerotomia. Reult The chirmer tet (P < .0001), break-up time (P < .0001), the Ferning tet (P < .001), and fluorecein and roe bengal taining (P < .0001) reulted in change in VKH. Biopy wa poitive in two patient and cintigraphy in four. Human leukocyte antigen DR4 wa found in 68.75% of VKH cae and in 83.3% of the cae with chirmer tet le than 3 mm. Concluion The incidence of keratoconjunctiviti icca wa higher in VKH yndrome patient than in control. Two patient atified the criteria for jögren yndrome, and two other had cintigraphy indicative of alivary gland involvement. The aociation of thee two autoimmune diorder i uggeted by the low incidence of VKH yndrome in Italy and might be related to HLA DR4.
