Abstract :
Purpoe
To compare retinal detachment a a reult of open and cloed globe trauma in a pediatric age group.
Deign
Retropective, comparative, conecutive, interventional cae erie tudy.
etting
Tertiary referral medical center.
Patient and method
One-hundred thirty-eight (5.7%) of 2,408 retinal detachment that were treated at our facility between 1980 and 2000 occurred in children aged 18 year or younger. Of thee, 37 eye (26%, n = 36) had retinal detachment following open globe injury and 23 eye (14%, n = 20) had retinal detachment following cloed globe injury. Thoe were compared with regard to the retinal detachment characteritic, number, type and timing of urgerie, and the anatomic and functional urgical outcome.
Reult
imilar incidence wa found in the type of retinal detachment, number of tear, extent, macular attachment type, and timing of urgery. Anatomic urgical ucce wa achieved in 16 eye (46%) with open globe injury and in 13 eye (65%) with cloed globe injury. The improvement in viual acuity wa limited and comparable in both group (23% to 25%), and lower than the expected according to the Ocular Trauma core (OT). The only predictor for favorable viual outcome of ≥20/200 wa preoperative macular attachment (P = .003, Fiher exact tet).
Concluion
The type, extent, and everity of the retinal detachment were imilar in both open and cloed globe injurie, uggeting that the detachment i caued by econdary indirect impact of globe deformation. The anatomic and functional urgical outcome wa guarded and imilar, uggeting that further urgical innovation i required to improve the viual outcome in thi age group.
