  • Abstract
    Purpoe To decribe and quantify the appearance of the optic dik in patient with advanced age-related macular degeneration (ARMD). Deign Retropective comparative tudy. Method A total of 316 chart were reviewed. From thee, 45 ubject with advanced ARMD (defined a geographic atrophy or diciform car) in at leat one eye were enrolled. Patient with glaucoma or glaucoma viual field defect were excluded. The probability of glaucomatou optic nerve damage wa cored by evaluation of tereocopic optic dik photograph in a maked fahion, according to a predefined evaluation cale and meaured with confocal ophthalmocopy (Heidelberg Retinal Tomograph; HRT). The area of macular involvement wa meaured in unit of dik area. The optic dik evaluation core and HRT meaurement of a group of eye with a large area of ARMD were compared with their fellow eye with maller area of ARMD. The correlation between the area of macular involvement with the optic dik evaluation core and HRT meaurement were analyzed. Main outcome meaure The optic dik evaluation core and the HRT meaurement between tudy group. Reult Eye with larger area of ARMD had optic dik that were more likely to be claified a glaucomatou by clinical evaluation (3.45 ± 0.23 v 3.05 ± 0.18, P = .015) and by HRT imaging analyi (−0.995 ± 0.423 v −0.376 ± 0.249, P = .030) than the fellow eye with maller area of ARMD. For eye with ix or more dik area of ARMD, larger area of ARMD correlated with a higher cup/dik ratio (P = .022), maller neuroretinal rim area (P = .022), and glaucomatou claification (P = .040) with HRT dicriminant function analyi. Concluion Eye with large area of geographic or dikiform ARMD have optic dik tructural alteration that reemble glaucomatou optic neuropathy. Thi hould be taken into conideration when evaluating patient with ARMD for glaucomatou damage
