Abstract :
Purpoe To decribe the characteritic and outcome of patient with cytomegaloviru (CMV) retiniti in the abence of human immunodeficiency viru (HIV) infection. Deign Retropective cohort tudy. Method Conecutive patient with CMV retiniti in the abence of HIV infection were identified at a univerity hopital. Demographic and clinical characteritic were noted at the time of CMV retiniti. Outcome were determined retropectively. Main outcome meaure were rate of econd eye involvement, viion lo, rhegmatogenou retinal detachment (RD), immune recovery uveiti, progreion of retiniti, and mortality. Reult The clinical characteritic of CMV retiniti in 18 patient (30 eye) without HIV infection diagnoed between January 1, 1984, and April 13, 2003, were imilar to thoe of patient with HIV infection. The incidence of viual lo to the level of 20/50 or wore and of 20/200 or wore were 17% per eye-year and 14% per eye-year, repectively. The oberved incidence of RD wa 3.7% per eye-year, and the mortality rate wa 23% per peron-year. Following reduction of immunouppreion, 10 patient (56%) who dicontinued anti-CMV therapy remained free of retiniti progreion. The incidence of immune recovery uveiti wa 13% per peron-year. Concluion In our erie, CMV retiniti in patient without HIV infection had a clinical coure imilar to that in patient with AID treated with highly active antiretroviral therapy (HAART), except the incidence of RD wa lower for patient without AID. A ubtantial number of patient no longer required long-term anti-CMV therapy after adjutment of immunomodulatory therapy.
