Abstract :
Uveal melanoma metatatic to the liver remain an incurable dieae. Unlike colorectal carcinoma metatatic to the liver, urgery i not conidered a firt-line approach becaue of the diffue, multifocal nature of the hepatic metatae. ytemic chemotherapy and immunotherapy regimen have had minimal efficacy for patient with metatatic ocular melanoma. Hepatic artery chemoembolization ha been explored to a limited degree, but report of repone remain anecdotal. Hepatic arterial infuion with fotemutine had an overall repone rate of 38%, but the overall urvival i till diappointingly hort. Iolated hepatic perfuion i a promiing technique for thi dieae. Although the reported repone rate i more than 60%, iolated hepatic perfuion i aociated with potential morbidity, and the number of patient treated by thi mean i till limited. Percutaneou hepatic perfuion ha the potential for le morbidity, and it efficacy in thi clinical etting warrant further evaluation. The concluion of thi review i that current treatment modalitie need further refinement and new one need to be developed to improve the outlook for patient with uveal melanoma metatatic to the liver. Unlike cutaneou melanoma, which metataize to numerou ite, uveal melanoma commonly metataize only to the liver. Thi undercore the importance of developing effective form of liver-directed therapy.—Han E. Groniklau
