Intraurgical plamin enzyme in diabetic macular edema

Purpoe To evaluate in a propective tudy the efficacy of intraurgical autologou plamin enzyme (APE) in producing poterior vitreou detachment. Deign Conecutive, interventional cae erie. Method A group of 12 eye of 11 patient with diabetic macular edema econdary to poterior vitreou cortex contraction wa conidered. A quantity of 0.1- to 0.2-ml containing 0.8 to 1.2 IU of APE prepared by our Coagulation ervice wa injected into the vitreou body 25 minute before urgery. The efficacy of the APE wa ubjectively evaluated. A control group of 10 eye with the ame clinical characteritic underwent the ame urgery without APE injection. Complete eye examination, including optical coherence tomography, were performed on all patient before urgery and during the 1-year follow-up period. Reult During urgery in the APE-treated group, the poterior vitreou cortex wa judged adherent in three cae, partially detached in ix cae, and totally detached in three cae. In two cae a complete collape of the vitreou body wa oberved. During urgery in the non–APE-treated group, the poterior vitreou cortex wa judged till adherent in nine of 10 eye and partially detached in 1 eye. Comparing the potoperative reult between the APE-treated group and the non-APE-treated group, we found no ignificant difference in final potoperative retinal thickne (P = .2552), wherea we found a ignificant difference in final viual acuity (P = .0121). Concluion Autologou plamin enzyme wa ueful in inducing a pharmacologic poterior vitreou detachment and in facilitating urgery. It did not eem to interfere with the final retinal thickne, and it ameliorate the final viual acuity.
