Abstract :
Purpoe
To ae whether baeline retinal nerve fiber layer (RNFL) meaurement obtained with a canning laer polarimeter, the GDx Nerve Fiber Analyzer, (Laer Diagnotic Technologie Inc., an Diego, California) are predictive of development of repeatable glaucomatou viual field damage in glaucoma upect eye.
Deign
Cohort tudy.
Method
Participant were recruited from the UCD longitudinal Diagnotic Innovation in Glaucoma tudy (DIG). One eye from each of 160 glaucoma upect with normal tandard automated perimetry (AP) viual field at baeline wa tudied. tudy eye were divided into convert and nonconvert group baed on the development of three conecutive glaucomatou viual field during follow-up. LP parameter, IOP, vertical cup dik ratio, tereophotograph aement a glaucoma or normal, corneal thickne, and viual field indice were included in univariate and multivariate Cox proportional hazard model to determine which LP RNFL and ocular parameter were predictive of viual field converion.
Reult
ixteen (10%) eye developed repeatable viual field damage (convert) and 144 (90%) did not (nonconvert). Mean (95%CI) follow-up time until viual field converion for convert eye wa 2.7 (1.7, 3.6) year. Mean total follow-up of nonconvert eye wa 3.8 (3.5, 4.1) year. Four out of thirteen examined baeline LP parameter and baeline AP Mean Deviation (MD), AP Pattern tandard Deviation (PD), and glaucomatou tereophotograph aement were ignificant univariate predictor of viual field converion. In multivariate model adjuted for age, IOP and CCT, LP parameter inferior ratio, ellipe modulation, and UCD linear dicriminant function (LDF) were ignificant predictor of viual field converion. When AP PD and tereophotograph aement were alo included in the multivariate model inferior ratio and UCD LDF remained independently predictive of viual field lo.
Concluion
Thinner baeline LP RNFL meaurement were independent predictor of viual field damage. In addition to thinner LP RNFL meaurement, higher baeline AP PD, and baeline glaucomatou tereophotograph aement each contributed to an increaed rik of the development of abnormal viual field in glaucoma upect patient. LP RNFL meaurement were independently predictive of future viual lo even when age, IOP, CCT, vertical cup dik ratio, and AP PD were included in the model.
