Abstract :
Purpoe To ae the hort-term afety and efficacy of treating ubfoveal choroidal neovacularization (CNV) with external beam radiation delivered in 5 × 4 Gy fraction among patient having age-related macular degeneration (AMD). Deign A multicenter propective randomized controlled pilot tudy. Method Eighty-eight patient were enrolled through 10 ite and were randomized to radiotherapy (20 Gy delivered in 5 daily fraction of 4 Gy each; 6 MV [N = 41]) or no radiotherapy (ham radiotherapy [N = 22] or obervation [N = 25]). Eligibility criteria included viual acuity of at leat 20/320 and ubfoveal CNV not amenable to treatment. Randomization wa tratified by leion type (new or recurrent CNV) and blood (<50% or ≥50% of the leion [N = 13]). The primary outcome meaure wa lo of ≥3 line of viual acuity. econdary outcome meaure were angiographic repone and ide effect. Reult At baeline, patient and ocular characteritic were imilar between treatment group. At ix month, 9 radiated eye (26%) and 17 eye not radiated (49%) lot ≥3 line of viual acuity (P = .04; tratified χ2 tet). At 12 month, 13 radiated eye (42%) and 9 oberved eye (49%) lot ≥3 viual acuity line (P = .60). The radiated group demontrated maller leion and le fibroi than the nonradiated group (P = .05 and .004, repectively) at 12 month. Radiation-induced complication were not oberved except for one radiated eye with numerou cotton wool pot and poible radiation retinopathy. Concluion External beam radiation at 5 × 4 Gy may have a modet and hort-lived (ix month) benefit in preerving viual acuity.
