Abstract :
Purpoe To compare primary par-plana vitrectomy (PPV) alone v vitrectomy with an encircling cleral buckling procedure for the treatment of primary rhegmatogenou peudophakic retinal detachment (PRD). Deign Propective, nonrandomized, comparative tudy. Method All 71 eye of 68 conecutive patient with PRD preented to our ervice between 1998 and 2002 were offered either vitrectomy alone (group-A) or vitrectomy in combination with encircling cleral buckling procedure (group-B). Preoperative and potoperative patient characteritic were recorded in detail. Main outcome meaure were reattachment with a ingle urgery, viual acuity, and reattachment urgery-related complication. Reult Retina reattachment with a ingle urgery wa achieved in 97.78% in group A and 92.31% in group B. Viual acuity improved by 3 or more line in 60% in group A and 69% in group B. Mean potoperative refractive error change (pherical) wa −0.05 diopter in group A and −1.43 diopter in group B. Potoperative intraocular preure on long-term follow-up wa elevated in 4.44% (group A) and 34.61% (group B). Average follow-up wa 12.45 month (±5.23 D) ranging from 9 to 40 month. We detected additional break intraoperatively in 54.9% of cae (both group). Concluion Vitrectomy i an effective initial treatment for PRD, wherea the benefit of an additional encircling buckling procedure i quetionable.
