Abstract :
Purpoe
To determine the effect on intraocular preure (IOP) of bypaing the trabecular mehwork in cultured human anterior egment.
Deign
Propective laboratory invetigation uing normal human eye obtained at autopy.
Method
Anterior egment from 21 eye were placed in perfuion culture, and trabecular bypa tent were inerted through the trabecular mehwork, with the lumen of the tube opening into chlemmʹ canal. Eye received from one to four tent, placed equiditant apart. In eye receiving one or two tent, additional tent were later added to a maximum of four per eye.
Reult
Intraocular preure wa lowered after placement of a ingle tent, from 21.4 ± 3.8 mm Hg to 12.4 ± 4.2 (P < .001). Thi correponded to an 84% increae in facility of outflow. Eye receiving more than one tent had final IOP of 11.9 ± 3.7 mm Hg. Nine eye had equential addition of tent, and even of thee had a further decreae of IOP (13.6 ± 4.1 to 10.0 ± 4.3; P = .02). Exciion of the entire mehwork, between tent, dropped IOP to 6.3 ± 3.2 mm Hg, indicating ome reidual mehwork or canal reitance remained even after placement of three tent.
Concluion
Bypa of the trabecular mehwork lower IOP in cultured human anterior egment. One tent produced the greatet change in preure. The equential addition of more tent further lowered preure in even of nine eye. Thi technique hold promie a a new clinical urgery for glaucoma.
