Abstract :
Purpoe
We report four patient with gaze-evoked amauroi attributable to incomplete poterior vitreou detachment and enuing vitreopapillary traction. We preent thee cae to illutrate and extend the pectrum of vitreopapillary yndrome and to draw attention to vitreopapillary traction and it expected manifetation in both optic dik appearance and optic nerve and retinal function.
Deign
Thi i a retropective obervational cae erie culled from tertiary neuro-ophthalmology practice.
Method
Patient were evaluated with direct and indirect ophthalmocopy, Hruby (precorneal) len, three-mirror Goldmann contact len, macular contact len, formal perimetry, fundu photography, fluorecein angiography, and orbital ultraound.
Reult
Four patient with gaze-evoked amauroi had dik edema aociated with a partial poterior vitreou eparation. Thee patient were young and had atypical poterior vitreou detachment characterized by periting vitreopapillary attachment.
Concluion
Gaze-evoked amauroi i a rare viual obcuration precipitated by change in volitional gaze, uually aociated with an underlying orbital ma. We extend it etiologie to implicate the vitreou through traction expreed at the optic dik. In our cae, vitreopapillary traction elevated the nerve head and eye movement precipitated tranient viual phophene followed by gaze-evoked amauroi caued by traction tranmitted from the vitreou to uperficial nerve fiber of the retina and dik.
