Abstract :
Purpoe To evaluate the clinical feature and urgical and viual outcome of rhegmatogenou retinal detachment (RRD) in the pediatric population. Deign Retropective, noncomparative, interventional cae erie Method A review of patient under 18 year who underwent primary retinal detachment urgery at National Taiwan Univerity Hopital from 1989 to 2003 wa conducted. Reult Included in the tudy were 152 eye of 146 patient (mean age of 13.1 year). Male patient compried 69.9% of the ample. Bilateral RRD wa preent in 4.1%. Etiologie included myopia >4 diopter (37.5%), trauma (32.9%), developmental anomaly (11.8%), previou urgery (5.9%), previou uveiti (3.9%), atopic dermatiti (2.6%), and unknown (5.3%). Macular detachment wa found in 73.0%. The mot common primary treatment wa cleral buckling (61.2%). ingle-operation reattachment wa accomplihed in 58.5% of patient and eventual reattachment in 78.3% of patient occurring in a mean of 1.5 (D = 0.9) operation. Average potoperative follow-up time wa 48.3 month. Viual improvement occurred in 42.8%, remained the ame in 32.2%, and worened in 19.1%. In the logitic regreion model, tatitically ignificant rik factor for poor urgical outcome were nonmyopic RRD (P = .026), macular involvement (P = .01), and preence of proliferative vitreoretinopathy (P = .07). Concluion Myopia >4 diopter wa the mot common etiology in retinal detachment in our ample, followed by trauma. Myopia (> −4.0 diopter) may be more common than previouly reported. Mot eye (78.3%) were anatomically reattached after multiple urgerie. Retinal detachment not aociated with myopia i a newly identified predictor for poor urgical outcome.
