Abstract :
Purpoe
To ae the efficacy and afety of ubconjunctival fluconazole treatment a an alternative therapy for evere fungal keratiti refractory to the conventional antifungal medical treatment.
Deign
Propective, interventional cae erie.
Method
Thi tudy included 13 eye of 13 patient (mean age 49.6 year, range 28 to 75) with evere fungal keratiti that did not repond to initial therapy with topical and intravenou fluconazole plu oral itraconazole. All patient were injected with ubconjunctival fluconazole 2%, up to 1.0 mL twice daily, for at leat 5 day. If neceary, therapy wa continued once daily for a maximum of 14 day after 5 day of injection.
Reult
ix eye were uccefully treated after 5 day of ubconjunctival injection of fluconazole. even patient required repeated injection for more than 5 day, but one of them finally underwent eviceration. The final viual acuity depended on the location of the remaining car; in four cae, corneal tranplantation wa neceary. Neither local nor ytemic toxic ide effect were oberved.
Concluion
ubconjunctival fluconazole could be effective for treatment of evere fungal keratiti and could be very ueful to avoid urgical intervention at an acute tage of thi infection. Although the efficacy of the variou doe cannot be determined from thi erie, the doe ued in thi tudy eem to be afe and effective for fungal corneal ulcer with hypopyon.
