Abstract :
Purpoe
The preent tudy decribe the pattern and predictor of treatment peritence and adherence among patient who are diagnoed with glaucoma or a glaucoma upect (baed on claim code).
Deign
A retropective cohort tudy uing health inurance claim data.
Method
Newly treated individual with diagnoed glaucoma (n = 3623) and upect glaucoma (n = 1677) were obtained from healthcare claim data in the Ingenix Reearch Databae. For each of thee two diagnotic group, we calculated the duration of continuou treatment with the initially precribed medication (peritence) and the prevalence of ue of the initial medication at variou time point (adherence). Four drug clae were included: β-blocker, α-agonit, carbonic anhydrae inhibitor, and protaglandin analog.
Reult
Nearly one half of the individual who had filled a glaucoma precription dicontinued all topical ocular hypotenive therapy within ix month, and jut 37% of thee individual recently had refilled their initial medication at three year after the firt dipening. Protaglandin were aociated with better peritence than any other drug cla, which wa indicated by hazard ratio for dicontinuation of protaglandin compared with β-blocker of 0.40 (95% confidence interval, 0.35–0.44) for diagnoed patient and 0.44 (95% confidence interval, 0.37–0.52) for patient with upect glaucoma. Protaglandin howed a imilar advantage in adherence. Furthermore, patient with diagnoed glaucoma were more likely to adhere to therapy than patient with upect glaucoma (relative rik = 1.11; 95% confidence interval, 1.05–1.18).
Concluion
Peritence and adherence were ubtantially better with protaglandin than with other drug clae, and patient with diagnoed open-angle glaucoma were more likely to adhere to treatment than upected glaucoma.
