Abstract :
Purpoe To determine clinical and demographic rik factor for corneal tranplantation in keratoconu patient. Deign Cae control tudy. Method etting: Tertiary referral center. tudy population: All keratoconic eye preenting to the Duke Univerity Eye Center from January 1, 1997, to December 31, 2002. obervation procedure and outcome meaure: Cae were defined a eye undergoing tranplantation during the tudy period for keratoconu. Control were defined a eye that did not undergo tranplantation. Cae were compared with control to determine clinical and demographic rik factor for corneal tranplantation. Reult A total of 131 eye with keratoconu were tudied; 59 cae were compared with 72 control. Group were imilar with repect to ex and race. ignificant univariate rik factor for corneal tranplantation were bet-corrected viion wore than 20/40 (OR 3.2, 95% CI 1.1 to 9.8), cylinder >10.0 diopter (OR 3.5, 95% CI 1.0 to 12.4), and average keratometry >55 diopter (OR 5.6, 95% CI 1.4 to 21.9). In multivariate analyi, age 30 or younger conferred a evenfold increaed rik of tranplantation compared with age of more than 40 (OR 7.7 for age 21 to 30 and OR 25.8 for age ≤20). Multivariate analyi alo howed duration of dieae 5 year or le and teep average keratometry a ignificant rik factor. Concluion A ubet of young keratoconic patient with high keratometry value may repreent an underrecognized rik group for corneal tranplantation. Bet-corrected viual acuity wore than 20/40 at preentation and atigmatim are other clinical indicator of increaed rik of corneal tranplantation in keratoconu patient.
