Abstract :
Purpoe
To analyze clinical feature of the tubulointertitial nephriti and uveiti (TINU) yndrome in Japan, epecially exacerbation or recurrence of uveiti.
Deign
Retropective obervational tudy.
Method
We reviewed the clinical feature in 12 patient with TINU yndrome diagnoed by renal biopy, who were evaluated and treated at the Department of Ophthalmology, Hokkaido Univerity Hopital. Viual acuity wa meaured uing decimal viual acuity.
Reult
Patient age ranged from 10 to 33 year (mean 21 year), and female accounted for 83% of the cae. Both eye were affected in 11 patient (92%). All 12 patient initially experienced ocular ymptom, with hyperemia being the chief complaint. We diagnoed patient a having uveiti and conducted blood tet and urinalyi. Urinalyi did how characteritic finding, mot notably glucouria, increaed β2 microglobulin that were ≥10 time that of normal level, and increaed N-acetylglucoaminidoe level in a high percentage of patient. Recurrent or exacerbating uveiti wa een in ix patient (50%). In exacerbated or recurrent uveiti, inflammation wa more evere (≥3+ cell of the anterior eye egment). The ocular inflammation generally reponded well to treatment with oral corticoteroid, and the number of recurrence wa lower at corticoteroid doe of ≥predniolone 40 mg.
Concluion
Ophthalmologit play an important role in the initial dicovery of patient with TINU yndrome. Becaue inflammation wa more evere in exacerbated or recurrent uveiti, in intance where uveiti i refractory to local therapy, oral corticoteroid hould be conidered a early a poible.
