Abstract :
Purpoe To evaluate the effect of treatment on the incidence of viion impairment and tructural ocular complication among patient with Vogt-Koyanagi-Harada (VKH) dieae. Deign Retropective cohort tudy. Method etting: ingle-center academic practice. tudy population: Twenty-four conecutive patient with VKH dieae who were referred for evaluation and treatment from November 1984 through May 2004. procedure: Information on patient with VKH dieae wa entered retropectively into a databae and analyzed with time-dependent technique. main outcome meaure: Incidence of viual impairment and of tructural ocular complication. Reult In better-eeing eye, the frequencie of viion lo to 20/50 or wore and to 20/200 or wore at preentation were 50% and 30%, repectively. The incidence rate on follow-up examination for viion lo to 20/50 or wore and to 20/200 or wore were 9% per peron-year and 7% per peron-year in the better-eeing eye. Oral corticoteroid therapy wa aociated with a reduced rik of lo of viual acuity to the 20/200 or wore in the better-eeing eye (relative rik [RR] = 0.33; P = .05). The ue of immunouppreive drug therapy wa aociated with a reduced rik of viion lo to the 20/50 or wore and to the 20/200 or wore threhold in the better-eeing eye (RR = 0.33; P = .05; and RR = 0.08; P = .04, repectively). Concluion VKH dieae may reult in ubtantial viual impairment. The ue of oral corticoteroid or immunouppreive drug therapy may reduce the rik of viion lo during the follow-up period.
