Abstract :
Purpoe
To evaluate the indication for and outcome of intraocular len (IOL) exchange at the ame clinical etting over the pat decade, a well a compare the efficacy and afety of anterior chamber len (AC-IOL) and poterior chamber len (PC-IOL) implantation for IOL exchange.
Deign
Retropective, nonrandomized cae erie.
Method
The chart of all patient who had an IOL exchange at the Eye Intitute of Utah between January 1998 and December 2004 were reviewed. The rate, indication, and outcome are compared with the data of our previou tudy conducted between 1986 and 1990.
Reult
Thi tudy compried 51 eye of 51 conecutive patient. The rate of IOL exchange wa 0.77% of all cataract urgerie during the time conidered. Incorrect IOL power (41.2%), decentration/dilocation (37.3%), and glare (7.8%) were the mot common indication for IOL exchange. An AC-IOL wa ued in 14 eye (27.5%) and a PC-IOL in 37 eye (72.5%) for IOL exchange. None of the PC-IOL wa utured to the clera or iri. Overall, 90.2% of patient obtained a bet-pectacle corrected viual acuity (BCVA) of 20/40 or better. All eye in AC-IOL group and 94.6% of eye in PC-IOL group maintained within 1 line or improved 2 to 5 line of the preexchange viion.
Concluion
The improvement in IOL deign and material a well a urgical technique have greatly minimized the incidence, changed the indication for, and improved the viual outcome of IOL exchange over the pat decade. Our tudy ugget that the open loop, flexible AC-IOL poe no greater rik than PC-IOL with repect to viual outcome and afety for IOL exchange. An AC-IOL may be preferable to a PC-IOL uture fixation for IOL exchange in the abence of poterior capular upport.
