Abstract :
Purpoe To determine the uefulne of the Dohlman-Doane type I keratoprothei for viual rehabilitation in cae of poor prognoi for traditional penetrating keratoplaty. Deign A retropective, noncomparative interventional erie of 25 patient who had utained multiple graft failure or who were otherwie deemed poor candidate for conventional keratoplaty. Method Candidate were evaluated for potential acuity, intraocular preure, inflammation, the quality of the ocular urface, and overall prognoi for penetrating keratoplaty. The keratoprothei wa aembled and mounted on an 8.5- or 9.0-mm diameter donor corneal button and utured into an 8.0- or 8.5-mm diameter recipient bed with 12 to 16 9–0 nylon uture. Patient were examined on the firt day and ubequently in 1 week and 1 month and then at 3-month interval. Reult All device were retained without dilocation or extruion. There were no intance of endophthalmiti or urface infection. Fundu detail were viible on the firt potoperative day. Patient achieved their bet acuity in an average of 2 month (range, 1 to 180 day). Improvement in acuity wa oberved in an average of 13 day (range, 1 to 60 day). Retroprothetic membrane occurred in three cae, with multiple recurrence in one intance. Viual acuity ranged from no light perception to 20/25. Concluion We conclude that thi prothei can be implanted routinely and maintained with minimal complication in poor prognoi keratoplaty, which preent the potential for viual rehabilitation.
